MFM Health — An Athenahealth Revenue Transformation: A Case Study

MFM Health is a multi-location primary care and specialty practice serving patients across Middleton, Danvers, and the North Shore of Massachusetts for more than 25 years. Under the leadership of Dr. Mark Allara, MFM delivers coordinated, community-based care across a broad range of clinical services using the athenaOne platform as its core clinical and financial system of record.

As the practice expanded across locations and service lines, leadership recognized that fully leveraging athenaOne’s revenue cycle, coding, and workflow capabilities would be critical to sustaining growth. MFM engaged Fedora Healthcare Solutions—alongside our regular collaborating partner and healthcare systems consultant Cory Legere Consulting—to modernize their Athena-based revenue cycle, tighten operational controls, and accelerate collections.

Key Challenges Identified

  • Charge capture delays driven by incomplete Athena workflows between provider sign-off and billing queues
  • Coding and diagnosis mismatches within athenaOne impacting claim scrubber performance
  • Authorization and referral gaps not consistently flowing into Athena claim records
  • Provider documentation and signature variability creating downstream billing delays
  • Imaging and ultrasound billing configuration issues (including incorrect modifier usage within Athena rules)
  • Lack of standardized Athena-based RCM workflows across locations

These gaps were not uncommon for a fast-growing, multi-location Athena practice—but they created unnecessary friction between care delivery and cash flow.

A Coordinated Athena Optimization Strategy

Fedora and Cory Legere partnered with MFM’s leadership, clinical teams, and IT staff to align clinical, operational, and financial workflows inside athenaOne.

Cory led efforts to optimize provider templates, clinical workflows, and Athena configuration, ensuring that documentation, orders, and authorizations were structured in a way that naturally produced clean, billable encounters. Fedora aligned those upstream improvements with revenue cycle execution, creating a closed-loop system from charting through payment.

Operational Improvements Implemented

Athena-Based Charge Capture & Billing Optimization

  • Implemented 24-hour charge submission once Athena notes were signed and locked
  • Standardized Athena coding rules, claim scrubbing, and payer-specific edits (CPT, ICD-10, LCDs, POS, contractor rules)
  • Established a structured Athena task and query workflow to rapidly resolve missing documentation and slips

Backlog Elimination

  • Identified and worked an Athena claim backlog of more than 4,000 encounters, bringing billing current within the first week

Athena AR & Denials Management

  • Centralized all Athena AR worklists, including aged balances, denials, and holds
  • Corrected posting and workflow issues to ensure every claim moved cleanly through Athena’s lifecycle
  • Partnered with MFM leadership to remove root causes of repeat Athena denials

Operational Improvements Implemented

Metric (Athena KPIs) Before Fedora After Fedora
AR > 90 Days 21% 14%
AR > 120 Days 16% 11%
Hold & Manager Hold Claims ~7,000 claims ~2,000 claims
Days to Bill (Athena) 6 days 3 days
Days in AR 38 days 31 days
Denial Rate 18% 7%

Key outcomes:

  • 25% increase in monthly collections
  • Over 60% reduction in Athena denials
  • Nearly 50% faster charge submission
  • Major reduction in aged AR and Athena worklist holds

A Flagship Athenahealth Success Story

By combining Fedora’s Athena-native revenue cycle execution with Cory Legere’s Athena configuration and clinical workflow expertise, MFM Health transformed athenaOne into a high-performance financial platform.

The result is a cleaner, faster, more predictable revenue cycle—fully aligned with how providers document care and how Athena is designed to operate—giving MFM a scalable foundation to support continued growth across the North Shore.

When we needed a trusted partner to get our Athena revenue cycle back on track, Fedora delivered. Their combination of platform expertise and operational rigor gave us confidence again in our billing and collections.
